Change of Address

All changes of address must be reported to the Warren City School District. When changes occur that impact your student's placement within the District (moving from one school to another), you are required to provide proof of new address (see examples below) to the Student Services office.

Examples of proof of address:
- Utility bills with top and bottom portion intact
- Current income tax form
- Bill of sale
- Rental agreement
- House insurance form
- Notarized statement from landlord
